In the event that the camper is not insured, any medical situation that would result in a need for payment would be self-pay.

I certify that I am aware of the inherent risks to my child associated with participating in camp activities, including archery, BB range, petting zoo, swimming and ropes course, and residing on camp property. I give my child permission to participate in all organized camp activities.

I agree to be financially responsible for any damage to or destruction of camp property, including, but not limited to, acts of graffiti by my child. I agree that in the event my child becomes a discipline problem, my child will be sent home at my expense and forfeit all monies paid.

The undersigned, whether rider, or parent/legal guardian of rider agrees to defend, indemnify and hold John Lea, Lauri Lea, OC Church DBA Generations Church, Sabine Creek Ranch Training Center, and its or their employees, agents, subcontractors, officers, directors, and owners (hereinafter collectively referred to as “Management”) harmless from and against any liability for any injury, loss or death suffered while on the premises or while engaged in any activity whatsoever or in any location whatsoever that is in any way associated with Management, and from and against any and all claims, losses, liabilities, attorneys’ fees, medical fees, costs and expenditures incurred by or asserted against Management, and from and against any and all claims including claims for injury or loss suffered, whether or not such injury or loss resulted directly or indirectly, from any reason including, but not limited to, the negligent or grossly negligent acts or omissions of Management. In other words, I agree to defend, indemnify and hold management harmless for its own negligence, contributory negligence and gross negligence. Furthermore, the undersigned agrees that Management shall not be liable or responsible for, and shall be indemnified and held harmless by the undersigned from and against any and all suits, actions, losses, damages, claims, or liability of any character, type or description, including all expenses of litigation, court costs, and attorney’s fees, for injury or death to any person or animal, or injury to any real or personal property, received or sustained by any person or persons, or animal or property, arising out of, or occasioned by, directly or indirectly, the use of the Premises, or any condition of the Premises or any other premises owned, leased or controlled by Management, including claims and damages arising in whole or in part from the negligence or gross negligence of Management, its agents, employees, servants, or any other person entering upon the premises.

I further give permission and consent to Sabine Creek Ranch Training Center, and Christian Camp and Conference Association (CCCA) for any photographs, videotapes and interviews to be taken during the camping session to be published and used to illustrate, report, promote and advertise the camp or CCCA including on Internet Web Sites promoting or reporting on the camp or CCCA. I hereby assign full copyright of these photographs to Sabine Creek Ranch Training Center with the reproduction either wholly or in part. I agree that they can be used separately or together, either wholly or in part, in any way and in any medium.

In the event that any one or more of the provisions contained in this Release shall be held to be invalid, illegal or unenforceable in any respect, that invalidity, illegality, or unenforceability shall not affect any other provision in this Release and this Release shall be construed as if such invalid, illegal, or unenforceable provision had never been included.

We authorize medical and surgical treatment or hospital care to be rendered to my child as needed in the judgment of the treating physician chosen by the Sabine Creek Administrator or an employee working under him/her.

Sabine Creek Ranch Liability Release

UNDER TEXAS LAW (CHAPTER 87, CIVIL PRACTICE AND REMEDIES CODE) AN EQUINE PROFESSIONAL IS NOT LIABLE FOR AN INJURY TO OR THE DEATH OF A PARTICIPANT IN EQUINE ACTIVITIES RESULTING FROM THE INHERENT RISKS OF EQUINE ACTIVITIES.

As a condition of being permitted to enter upon the Sabine Creek Ranch Training Center premises located at 576 Sabine Creek Road, Royse City, TX 75189 (hereinafter the "Premises") and/or being permitted to participate in
equestrian, waterfront or other recreational activities, I hereby make
the following declaration and agreement:

I, the registrant or my legal guardian (hereafter called "registrant", along with
the Sponsoring Organization (if any), its attendees and the undersigned
understand and are fully aware that equestrian sports, all related
activities involving horses, specifically including but not limited to
horseback riding and trail rides, hayrides and rural activities
involve inherent dangerous risk of serious injury or death,
and that by participating in such activities the registrant or their legal guardian, Sponsoring Organization, its attendees and the undersigned expressly assume any and all risks of injury or loss and full responsibility for their themselves and their guests for any accident or personal injury or
property damage that may occur while utilizing Sabine Creek Ranch Training Center facilities, recreation events, camp activities, transportation services or anything associated therewith.

The registrant, Sponsoring Organization, its attendees and the undersigned hereby release, indemnify and hold harmless John Lea, Lauri Lea,
OC Church DBA Generations Church, Sabine Creek Ranch Training Center, and their employees, agents, representatives, officers, directors, and
owners (hereinafter collectively referred to as "Management") from and
against any and all claims, liability, suits, actions, costs, attorneys' fees, damages, and loss, whether associated with personal injury or property damage.

By clicking "I agree" I hereby represent that I have full capacity and
authority to enter into this agreement on behalf of: myself in the case of an
adult participant; my minor child in the case of a minor participant; or the Sponsoring Organization.

In the event that any one or more of the provisions contained in this
Release shall be held to be invalid, illegal or unenforceable in any
respect, that invalidity, illegality, or unenforceability shall not
affect any other provision in this Release and this Release shall be
construed as if such invalid, illegal, or unenforceable provision had
never been included. Sabine Creek Ranch Training Center reserves the right to refuse registration to any person at its sole discretion. Guests who do not follow the ranch rules or who are disruptive may be removed from the premises with no refund.

I have read, understand, and agree to the terms contained in the Sabine Creek Ranch Training Center Liability Release Form, and acknowledge this on behalf of myself and any minors for whom I am responsible by agreeing and continuing the registration process. If you do not agree, registration will not be complete, and no space will be reserved.

I acknowledge that all transportation (arrival/departure/early leave) will be coordinated with the organizational group with which my camper is attending. Any early departures will need to be approved by the affiliated group and campers will need to be signed out in the Sabine Creek Ranch camp office.
